FIELD: medicine.
SUBSTANCE: invention relates to medicine, in particular to oncology and thoracic surgery. The lesser curvature of the stomach is mobilized. An isoperistaltic gastric graft is formed. The esophagus is resected. An anastomosis is formed on the neck. At the same time, the medial leg of the sternocleidomastoid muscle is cut off from the clavicle. The muscle flap is separated from the main muscle bundle and rotated posteriorly. Then, with two U-shaped sutures, the distal part of the muscle is fixed to the vertebral fascia. The muscle is evenly placed in the space between the graft and the spine. The graft is displaced anteriorly until it adheres to the thyroid gland and the neurovascular bundle. After that, drainage is performed through the counter-opening and connected to a vacuum. The skin is sutured with interrupted sutures.
EFFECT: method allows performing effective fixation of the graft to the neck structures, preventing the formation of a "channel" around the graft and the esophagus, connecting the anastomosis area and the mediastinum, minimizing the risk of mediastinitis in case of failure of esophagogastroanastomosis on the neck.
