FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely surgery, endoscopy, toxicology. On the 2nd day after receiving a chemical burn, esophagogastroduodenoscopy and endosonography of the esophageal wall with visualization of the necrotic mucous membrane of the esophagus are performed. When ulcerative burn lesion of the esophagus is detected with damage to the muscle layer of the esophageal wall of 0-2 degrees according to the Y. Kamijo classification, mechanical removal of the necrotic mucous membrane of the esophagus is performed using an endoscopic direct distal transparent cap under visual control. At the same time, necrotic tissues are “pushed” by the side of the cap, shifting the endoscope in the distal direction. The necrotic mucous membrane of the esophagus in the esophagus wall is removed, followed by application of autologous plasma rich in platelets and a solution of type 1 allogeneic collagen to the area of the cleared defect of the esophageal mucosa.
EFFECT: method improves the effectiveness of endoscopic treatment of chemical damage to the esophageal wall, reduces the risk of bleeding and the formation of esophageal stenoses, as well as reduces inflammation and early epithelialization of damaged areas of the esophageal wall, reduces the time of endoscopic examination, reducing the complexity of treatment for both the patient and the doctor, and also reduces the number of necessary endoscopic studies.
