FIELD: medicine; surgery; endoscopy; toxicology.
SUBSTANCE: on the 3rd day after receiving a chemical burn, esophagogastroduodenoscopy is performed with visualization of the damaged mucosa of the esophagus and endosonography of the esophageal wall. The degree of damage to the muscular layer of the esophageal wall is determined according to the Y. Kamijo classification. Based on the results obtained, as well as depending on the type of corrosive substance, the following endoscopic treatment is chosen: 1) mechanical removal of the necrotic mucosa of the esophagus using an endoscopic direct distal transparent cap under visual control, followed by the application of autologous platelet-rich plasma and a solution of type 1 allogeneic collagen; 2) injection of a plasma-free lysate of autologous platelets along the proximal and distal edges of the damaged area around the circumference of the esophagus, followed by injection of a solution of type 1 allogeneic collagen under the necrotically altered tissues of the esophagus; 3) percutaneous endoscopic gastrostomy before the expiration of 2 weeks from the time of receiving a chemical burn.
EFFECT: method allows to increase the effectiveness of endoscopic treatment of chemical damage to the esophageal wall, reduce the risk of bleeding and esophageal stenosis, and restore adequate enteral nutrition in the early stages by formation of extended decompensated esophageal stenosis.
