SUBSTANCE: invention relates to medicine, namely laparoscopic surgery. 12 hours before the preoperative CT scan of the abdominal cavity, the patient takes orally a 6.4% aqueous solution of macrogol 4000 at the rate of 1 liter per 20 kg of body weight, dosed with 250 ml every 15 minutes. Then, after complete bowel emptying, the patient undergoes a polypositional computed tomography with bolus contrast enhancement in the back position and in the operating position, i.e. on the left side at an angle of 90° to the surface of the operating table. If in the position of the patient on the left side in the axial reconstruction of computed tomography, the displacement of the liver with a pathological focus anteriorly is 6 cm or more compared to the position on the back, then the point of introduction of the optical trocar is set in the right hypochondrium along the midclavicular line, and with a displacement of less than 6 cm in the right hypochondrium along the anterior axillary line.

EFFECT: method is effective, technically simple, feasible in any hospital equipped with a CT scanner, reduces the time of surgical intervention, improves the ergonomics of surgical manipulations, reduces the likelihood of iatrogenic complications, especially at the stage of mastering the technique of laparoscopic operations on the VII and VIII segments of the liver.
