FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to oncology. The markings are made on the skin in the form of a rhombus, as in Fig. 1, and the skin of the perineum is resected along the midline from point 1 - the upper edge of the tendon center to point 2 - the middle of the distance between the anatomical localization of the posterior commissure and the anus. Incisions are made on the skin in the area of the projection of the ischial tuberosity on the right - point 3 and on the left - point 4, one surgical monofilament polydioxanone thread M 3.5, USP 0 is applied with two atraumatic round piercing needles with a length of at least 65 mm, curvature ½, completely smooth , with a resorption period of 180-240 days along a diamond-shaped trajectory: using one of the two needles, the thread is carried out from point 1 with the capture of the right transverse perineal muscles and punctured at point 3, then from point 3 to point 2, after which, using another needle, from point 1, with the capture of the left transverse perineal muscles, to point 4, then from point 4 to point 2. Next, the perineum is tightened until the defect of the genital fissure is closed and the anatomy of the pelvic floor is restored, the thread is tied into a knot, after which a cosmetic suture is applied over the knot on the resected skin of the perineum using polydioxanone thread, size M 1.5, USP 4/0. Stabilized hyaluronic acid with a density of 20 mg/g is submucosally injected into the mucosa of the upper, middle and lower thirds of the vagina, and after preliminary installation of a Foley catheter, stabilized hyaluronic acid with a density of 25 mg/g is paraurethrally injected into the bladder with a density of 25 mg/g at 4 points - at 3, 6, 9, 12 conventional clock face.
EFFECT: method provides not only effective restorative treatment of involution- and post-radiation-altered tissues of the genitourinary system, but also prevention of the development of severe post-radiation complications due to the restoration of its anatomical and functional properties, and also improves the quality of life of patients, which is the second most important criterion for the effectiveness of antitumor therapy after survival.
