SUBSTANCE: invention relates to the field of medicine, namely to the field of traumatology, and can be used for surgical treatment of unstable associated injuries of the elbow joint using a temporary dynamic fixator. In the preoperative period, the nature of damage to the bone structures of the elbow joint is determined by radiography and the anatomical features of the structure of the patient's injured elbow joint are determined. After performing anesthesia under a tourniquet on the shoulder in the position of the patient on the back, external access is made to the affected elbow joint of the patient. The capsule is opened and partial cutting of the common extensor tendon from the lateral epicondyle of the shoulder is performed. During revision of the elbow joint, hematoma and blood clots are removed, osteosynthesis of the coronoid process is performed, and, depending on the nature of the fracture of the head of the radius, extra- or intracorporeal reposition and osteosynthesis are performed. The joint is stabilized by installing a temporary dynamic fixator made of a metal wire with a diameter of 1.6 mm, bent in the form of a crossed closed loop with bent pointed ends, a point of the rotation center is selected on the lateral surface of the head of the condyle of the humerus, a hole of a closed loop of a temporary dynamic is placed above the selected point. fixator and fix the temporary dynamic fixator through the hole of the loop with a screw, while the pointed ends of the temporary dynamic fixator are inserted into the ulna. The amplitude of passive movements and the stability of the joint during movements are monitored after placement of a temporary dynamic fixator, which is removed 3-4 weeks after surgery. In some cases, if there is a dislocation of the forearm, it is reduced, if the radial collateral ligament is damaged, a suture is applied to it. Temporary dynamic retainer and fixing screws are made of titanium alloy or medical grade stainless steel.
EFFECT: method provides reliable anatomical restoration of the shape and support function of the elbow joint with simultaneous absence of multiplanar displacements of the fixed bones, preservation of the patient's motor activity in the early postoperative period, provision of the necessary amplitudes of passive movements and stability of the elbow joint during movements, as well as improving the patient's quality of life due to the use of a temporary dynamic lock.
