FIELD: medicine; traumatology and surgery.
SUBSTANCE: invention can be used for surgical treatment of posterolateral rotational instability of the elbow joint. The method includes the following 3 stages: at the first stage, an arcuate skin incision is made along the supracondylar crest of the humerus, extending distally 5 cm posterior to the head of the radius, the lateral part of the condyle of the humerus and the lateral capsule of the elbow joint are visualized. At the second stage, bone tunnels are formed as follows: two holes are drilled in the ulna, the first — near the tubercle on the crest of the supinator, and the second — 1–2 cm proximally at the base of the annular ligament, between which a tunnel is formed using a curved awl, then in isometric point of attachment of the lateral ulnar collateral ligament (ULCL) on the lateral epicondyle of the humerus, a blind canal with a depth of 15 mm is formed. At the third stage, the autograft is performed and fixed as follows: the autograft is passed through a tunnel in the ulna in a position of flexion of the elbow joint from 30 to 40 degrees with pronation of the forearm. The length of the free ends of the autograft is measured and shortened so that 15 mm of the autograft enter the canal of the humerus. The free ends of the autograft are passed into the canal in the humerus and fixed with an interference screw in a position of flexion of the elbow joint from 30 to 40 degrees and with pronation of the forearm. The method simplifies the anatomical restoration of the lateral ulnar collateral ligament, increases the stability of fixation of the autograft on the humerus, reduces the time of immobilization and accelerates the recovery period due to the achievement of maximum isometry in the elbow joint due to the peculiarities of graft fixation.
