SUBSTANCE: invention relates to medicine, namely to ophthalmology, and can be used to select tactics for the treatment of central serous chorioretinopathy (CSCR) complicated by type 1 pachychoroidal neovascularization based on preoperative spectral optical coherence tomography (SOCT) data - preoperative SOCT predictors. To do this, a study is carried out using the method for spectral optical coherence tomography (SOCT), during which the height of the detachment of the neurosensory retina (NSR), the thickness of the choroid, the length of the detachment of the retinal pigment epithelium (RPE) above the zone of choroidal neovascularization (CNV) type 1 are determined. In the projection of the zone of presence according to SOCT data of fibrovascular detachment of RPE, a study is performed in the angio mode and the area of CNV type 1 is determined. When the thickness of the choroid is more than 470 µm, the area of CNV type 1 is less than 0.27 mm2, the length of the RPE detachment over the zone of CNV type 1 is less than 1.3 mm, the height of the NSR detachment is more than 310 µm, combined treatment is considered indicated, including subthreshold micropulse laser exposure (STMLE) over the entire area of detachment of the NSR and in the projection of the zone of CNV type 1, excluding the area above the foveola, one day before the intravitreal injection of an angiogenesis inhibitor (IVIAI). If the thickness of the choroid is less than 470 μm, the area of CNV type 1 is more than 0.27 mm2, the length of the RPE detachment over the zone of CNV type 1 is more than 1.3 mm, the height of the subfoveolar detachment of the NSR is less than 310 μm, IVIAI therapy is considered indicated.
EFFECT: invention provides the choice of an effective method for the treatment of CSCR complicated by CNV type 1: combined treatment, including STMLE throughout the entire zone of HCC detachment and in the projection of the type 1 CNV zone, excluding the area above the foveola, one day before IVIAI, or therapy with intravitreal angiogenesis inhibitor administration - based on preoperative SOCT data that determine the cause of NSR detachment: CNV activity Type 1 or exacerbation of CSCR - with the achievement of resorption of SRF, adherence of the NSR, improvement of functional results.
