METHOD FOR ENDOSCOPIC DECOMPRESSION AND DISPLACEMENT OF THE ULNAR NERVE TO THE ANTERIOR SURFACE OF THE FOREARM Russian patent published in 2023 - IPC A61B17/00

Abstract RU 2789265 C1

FIELD: health care.

SUBSTANCE: invention relates to health care, namely to neurosurgery, and can be used in the treatment of tunnel neuropathy of the ulnar nerve at the level of the cubital canal. An incision of the skin and subcutaneous fat is made in an arcuate shape, 3 cm long, in the projection of the medial condyle. Using a tupfer, a space is formed in the projection of the ulnar nerve and the medial surface of the shoulder and forearm, the fascia is opened at the level of the cubital canal, the ulnar nerve is visualized. An endoscope is inserted in the proximal direction and, under its control, the upper part of the cubital canal formed by the Strazer arcade is opened. Decompression of the ulnar nerve to the lower third of the shoulder is performed. The endoscope driven in the distal direction and the Osborne ligament is opened under his control. Decompression of the ulnar nerve to the upper third of the forearm is performed. A contraperture is formed in the ulnar fossa. The medial condyle with the tendon part of the anterior muscle group of the forearm is cut off in the sagittal plane. The wrist flexor muscles and muscles of the fingers from the ulnar bone for 3 cm are mobilized. Pulling up the holder, the ulnar nerve is driven under the severed medial condyle and intermuscular septum, the condyle is placed at the place of its attachment and fixed with 2 screws. The intermuscular septum is sutured to the place of its attachment and fixed with 2-3 stitches. The wound is sutured in layers tightly.

EFFECT: present invention enables to prevent relapses of the disease and the preservation of the functional state of the limb due to the fact that the nerve is covered with muscles, preventing the prerequisites for migration and the formation of a false joint in the region of the condyle.
