SUBSTANCE: invention relates to medicine, namely to radiation diagnostics, and can be used for differential diagnosis of the presence or absence of splenomegaly and hypersplenism syndromes in patients with hepatitis and liver cirrhosis. Magnetic resonance imaging of the spleen is carried out to determine its size, characterized in that then the proton spin marking mode is carried out by a magnetic field in arterial blood water molecules (ASL-perfusion) of the spleen, with a quantitative assessment of perfusion in the parenchyma. If the size of the spleen is enlarged, the ASL - perfusion rate is ≥259 ml/100 g/min, then this indicates the presence of splenomegaly syndrome. If the size of the spleen is enlarged or not enlarged, the ASL - perfusion rate ≤246 ml/100 g/min, then this indicates the presence of hypersplenism syndrome. If the size of the spleen is not enlarged, the ASL-perfusion rate is from 247 to 258 ml/100 g/min, then the absence of splenomegaly and hypersplenism syndromes is judged.

EFFECT: method provides differential diagnosis of splenomegaly and hypersplenism syndromes in patients with hepatitis and cirrhosis of the liver due to the quantitative assessment of perfusion in the parenchyma.
