FIELD: medicine.
SUBSTANCE: invention can be used in pulmonology to assess microcirculatory disorders as a biomarker of interstitial changes in the lungs of patients who have undergone a new coronavirus infection. Capillaroscopy of the nail bed is carried out. The following is determined: the density of the capillaries of the distal row, the shape of the capillary loops and abnormally shaped capillaries — spiral, crossed or twisted, the presence of enlarged capillary loops with a capillary loop diameter of 20–50 microns and giant capillary loops with a loop diameter of more than 50 microns, microhaemorrhages and hemosiderin deposits in the capillary zone, the length of the pericapillary zone, as well as the presence of stasis. Each of the defined indicators is assigned a score in points. With 7 or more capillaries per 1 mm, 0 points are assigned, with 7–5 capillaries per 1 mm 1 point is assigned, with 5 or less capillaries per 1 mm 2 points are assigned. If capillary loops are determined, either similar to a hairpin, or U-shaped and no more than 25% of the capillaries are of an abnormal shape, 0 points are assigned, if up to 50% of the capillaries are of an abnormal shape 1 point is assigned, if more than 50% of the capillaries are of an abnormal shape and/or presence of bushy capillaries is determined 2 points are assigned. In the absence of enlarged and giant capillary loops, 0 points are assigned, in the presence of single enlarged capillary loops 1 point is assigned, in the presence of multiple enlarged capillary loops and/or the presence of giant capillary loops 2 points are assigned. In the absence of microhaemorrhages and hemosiderin deposits in the capillary zone, 0 points are assigned, in the presence of single microhaemorrhages and hemosiderin deposits in the capillary zone 1 point is assigned, in the presence of multiple microhaemorrhages and hemosiderin deposits in the capillary zone and/or confluent haemorrhages 2 points are assigned. If the length of the pericapillary zone is up to 100 mcm, 0 points are assigned, if the length of the pericapillary zone is more than 100 µm 1 point is assigned, if there is a pronounced near-capillary oedema that makes visualization difficult 2 points are assigned. In the absence of stasis, 0 points are assigned, in the presence of single stasis 1 point is assigned, in the presence of large multiple stasis 2 points are assigned. Sum up all the points. If the total score is more than three, a high probability of the presence of interstitial changes in the lungs after the disease caused by a new coronavirus infection is determined.
EFFECT: determining the risk of development and progress of interstitial changes in the lungs under conditions of a new coronavirus infection due to the stated diagnostic criteria.
