SUBSTANCE: method for restoring the extensor apparatus of the knee joint during extensive resections of the proximal tibia with endoprosthetics performed for a tumour lesion, including replacing the post-resection defect of the tibia with an endoprosthesis with non-free plasty with an axial muscle flap from the medial head of the gastrocnemius muscle, characterized by fixing a part of the titanium mesh on the proximal module of the tibial component of the endoprosthesis, the mesh should be sufficient in length to make one complete revolution around the endoprosthesis module, a part of the mesh is left unattached, then the patellar ligament remaining after tumour resection is sutured to the fixed part of the titanium mesh, which is pulled to full extension in the knee joint during contraction quadriceps femoris muscle, then the patellar ligament is covered with the remaining part of the titanium mesh cloth, after which all layers of the titanium mesh and its own ligament at the patella are stitched together, and then covered from the outside with a transplanted axial muscle flap from the medial head of the gastrocnemius muscle.
