METHOD OF FEMORAL-POPLITEAL SHUNTING WITH A NARROWED DIAMETER OF THE GREAT SAPHENOUS VEIN Russian patent published in 2023 - IPC A61B17/00 A61B17/11 A61F2/06 

FIELD: medicine; vascular surgery.

SUBSTANCE: at the 1st stage, a section of the great saphenous vein (GSV) is isolated, ligated and crossed. A bed is formed in the subcutaneous fat on the anteromedial surface of the thigh at the level of the middle of the length of the planned shunt. Then the GSV is reversed and placed in the created bed in the direction of the femoral artery. An arteriovenous fistula is formed between the common femoral artery and the distal end of the reversed great saphenous vein. After 3–4 weeks, the second stage, after an increase in the diameter of the narrowed vein-fistula from 3 mm or more, confirmed by triplex scanning, crossectomy is performed. The proximal end of the autovein is reversed and an anastomosis is formed between the popliteal artery and the proximal portion of the autovein.

EFFECT: method allows to reduce the risks of shunt thrombosis in femoropopliteal shunting due to the use of a reversed GSV of sufficient diameter as a shunting material, to reduce the time and volume of surgical intervention, to shorten the rehabilitation period and to reduce the risks of infectious complications due to the absence of additional incisions on the arm.
