FIELD: medicine, cardiology, X-ray endovascular methods of diagnosis and treatment.

SUBSTANCE: puncture of the right and left femoral veins is performed, introducers are installed according to the Seldinger method, a sensor for intracardiac echocardiography (IC-ECHO) is inserted into the right atrium. Afterwards, a multi-purpose catheter is introduced into the right atrium along the diagnostic conductor. Then, a diagnostic conductor is inserted through the open foramen ovale (OOO) into the cavity of the left atrium, after which, in the left upper pulmonary vein, through a multi-purpose catheter, the diagnostic conductor is replaced with a super-rigid conductor, through which a delivery system with an occluder is inserted into the cavity of the left atrium. Further, in the cavity of the left atrium, the occluder disc is opened, the occluder is pulled up to the septum, and the second occluder disc is opened in the cavity of the right atrium. After that, a tilt test and a bubble test with an agitated solution are performed, the procedure is carried out under the control of VS-ECHO without the use of fluoroscopy.

EFFECT: method allows to perform intervention in difficult situations when additional visualization of the interatrial septum is required, is well tolerated by the patient in comparison with transesophageal echocardiography, does not require sedation and intubation, and also reduces the operation time, performs intraoperative monitoring of the results of the intervention, such as the presence of residual shunt in the open oval window, bubbles in the left sections during the Valsalva maneuver, in addition, it allows to reduce the radiation exposure to the patient and staff.
