FIELD: medicine; cardiology.
SUBSTANCE: invention relates to X-ray endovascular diagnostics. Peripheral artery is punctured, an introducer is inserted, followed by insertion of a diagnostic catheter into a left ventricular cavity. Ventriculography is performed to determine the size and location of the ventricular septal defect under transthoracic echocardiographic control. Thereafter, a conductor is delivered through an arterial approach from a left ventricular cavity through a ventricular septum defect into a superior/inferior vena cava cavity, and a diagnostic catheter is inserted. Through the venous access into the superior/inferior vena cava cavity, a delivery system with an occluder is introduced through an artery using a diagnostic catheter into the superior/inferior vena cava cavity, a loop-trap system is introduced, 1/3 of the left disc of the occluder is released from the delivery system, followed by fixation of the left disc of the occluder. Occluder–trap–catheter complex is then introduced into the delivery system and inserted into the left ventricular cavity. Thereafter, the delivery system is pulled along the occluder into the right ventricle, and the right disc is opened in the right ventricle. Left disc remains in the left ventricle in the folded position fixed by the trap. Thereafter, X-ray and transthoracic echocardiography is followed by opening and positioning the left disc of the occluder and releasing it from the loop of the trap system. Then, after control ventriculography and transthoracic echocardiography, the right disc of the occluder is disconnected from the delivery system.
EFFECT: method enables achieving optimal positioning and opening of the occluder on the first attempt, which enables to avoid the development of life-threatening cardiac rhythm disturbances and destabilization of central hemodynamics.
