FIELD: dentistry.
SUBSTANCE: invention is intended for use in the restoration of complete loss of teeth in the lower jaw with orthopedic structures. Cone-beam computed tomography with a contrast agent is performed. The received three-dimensional images are converted into three-dimensional models of the jaw. The resulting images are compared in the virtual space of the 3D graphics software. Three-dimensional modeling of the basis of the prosthesis is carried out, the manufacture of a prototype prosthesis by means of three-dimensional printing of virtual simulated objects, fitting, coordination and selection of the prototype prosthesis in the patient's oral cavity, for which the patient's oral cavity is filled with a volume of contrast agent and, holding in the mouth, without swallowing, is sent for shooting, and when processing the obtained images of the digital model of the lower jaw, the areas of the location of the compression zones are determined, excluding injury and ischemia of the soft tissues in the area of the border of the prosthetic bed, which are taken into account when obtaining an individual relief of the prosthetic bed of the lower jaw by performing correction of the boundaries and subsequent production of a prosthesis prototype. A hydrophobic pasty paste is used as a contrast agent, obtained by mixing Vaseline pharmaceutical preparation and a powder based on barium sulfate at a mass ratio of components of 1:4, and when processing the obtained images of a digital model of the lower jaw, the boundaries of the prosthetic bed are determined, bypassing the places of muscle attachment the bottom of the mouth — the area of the muscleless triangle in the distal part of the maxillary-lingual groove, thickening of the base for adhering to the functionally mobile areas of the prosthetic bed, hollow pockets for creating a gap in the area of the internal and external oblique lines, valvular zones located in the area of retromolar mucous tubercles.
EFFECT: due to the identification and further use of additional retention points, taking into account individual anatomical and topographic features of the structure of the maxillary-lingual groove, the method allows to minimize trauma to the soft tissues of the prosthetic bed and increase anatomical retention, fixation of a complete removable denture and functional efficiency.
