FIELD: medicine; pancreatobiliary surgery; oncology.
SUBSTANCE: following risk factors are determined: male gender; smoking; diameter of the main pancreatic duct <3 mm; density of the pancreas in the isthmus less than +30 HU; thickness of the pancreas measured as the anteroposterior size of the pancreas on the sagittal section during preoperative CT in the projection of the left edge of the superior mesenteric artery of 12 mm or more; body mass index over 25 kg/m2. If three or more risk factors are detected, a high risk of postoperative pancreatic fistula development is determined, and at the preoperative stage the patient undergoes endoscopic papillosphincterotomy with stenting of the main pancreatic duct with a preliminary determination of the stent length, which corresponds to the distance between the major duodenal papilla and the left semicircle of the superior mesenteric vein.
EFFECT: method provides prevention of postoperative pancreatic fistula development, reduces treatment time, improves the patient's quality of life.
