FIELD: medicine; cardiac surgery.

SUBSTANCE: right-sided mini-thoracotomy access is made in the chest. Excision of the aortic valve leaflets and aneurysmally altered tissues of the ascending aorta up to the fibrous ring of the aortic valve is performed. A 3–4 mm border of the aortic tissue is left and the orifices of the coronary arteries are cut out according to the type on the sites. Then subsequent fixation of the linear vascular prosthesis over the rim of the aortic tissue is performed with a two-row blanket suture and reimplantation of the coronary artery orifices. Next, the seamless biological valve is positioned by the delivery system for its implantation under vision control into the previously fixed linear vascular prosthesis.

EFFECT: method allows to reduce the total time of the operation to replace the aortic root in patients with high surgical risk, as well as to reduce the time of cardiopulmonary bypass and aortic clamping; perform safe aortic valve replacement if its annulus fibrosus is affected by calcification or infective endocarditis.
