FIELD: medicine; cardiovascular surgery.

SUBSTANCE: invention can be used in surgical management of ascending aortic aneurysm in combination with aortic valve defect. In place of the hole for the anastomosis, a rectangular flap on a wide pedicle is cut out in the wall of the prosthesis by dissecting three edges, the upper one—transversely, two lateral incisions are made at distance of 3–6 mm from the edges of the coronary ostium. Lateral incisions are stopped short of the artificial valve cuff while maintaining the integrity of the lower edge as a pedicle. Flap is sutured to the aorta from the angle farthest to the surgeon with continuation of the suture to the sub-ostium part of the aortic wall. Wall of the aorta is pierced through all layers, perpendicular to the ostium, and the needle is pricked out, not reaching the edge of the ostium; the same distance is observed between the stitches. Upon reaching the opposite angle, the suture along the edge of the flap is turned upwards and the place of transition of the edge of the flap to the edge of the hole on the prosthesis is passed, working in turn with both needles, suture is completed at the middle of the upper edge of the hole, above the coronary ostium by 3–6 mm.

EFFECT: method provides higher reliability of sealing at low-set coronary ostia and eliminates tension of suture line.
