METHOD OF SUTURING THE RESIDUAL BONE CAVITY AFTER REMOVAL OF IMPACTED LOWER THIRD MOLAR Russian patent published in 2023 - IPC A61B17/24 A61C3/14 

FIELD: medicine; surgical dentistry.

SUBSTANCE: vestibular mucoperiosteal flap is mobilized. The edge of the flap adjacent to the distal surface of the second molar of the lower jaw is deepithelized. Then a needle with a thread is injected into the base of the gingival papilla from the vestibular surface between the first and second molars of the lower jaw on the side of the intervention. Next, a puncture is made from the lingual surface of the gingival papilla. Then, an injection is made through the lingual mucosa at the level of the distal surface of the second lower molar, departing 2–3 mm apically from the gingival margin. The vestibular flap is pierced through in the deepithelization zone from the inner surface, stepping 2–3 mm back from the edge. Injection is performed through the mucosa lingually at the level of the distal surface of the lower second molar. An injection is made into the base of the gingival papilla from the lingual surface between the first and second molars of the lower jaw. Next, a puncture is made from the vestibular surface of the gingival papilla at a distance of 3–5 mm from the free edge of the thread. The free threads are simultaneously tensioned by pulling the edge of the vestibular flap under the mucosa from the lingual surface in the region of the second molar of the lower jaw. A surgical knot is tied. On the distal part of the incision, 1 simple interrupted suture or 2 simple interrupted sutures are placed at a distance of 3 mm from each other.

EFFECT: method makes it possible to ensure hermetic suturing of the residual bone cavity after removal of the retained third molar of the lower jaw, healing of the mucosa by primary intention, which is necessary when performing a coronectomy operation or performing bone grafting of the residual bone cavity.
