FIELD: neurosurgery.
SUBSTANCE: topography of the basilar artery relative to the tumor of the ventral posterior cranial fossa is determined. The presence and volume of petrification in the tumor is determined using a computed tomogram. In case of petrification of the tumor by 40 percent or less relative to its volume, the tumor is removed by endoscopic endonasal transclival access by fragmentation in the presence of one of the options for the location of the basilar artery relative to the tumor: posterior, posterolateral to the right, posterolateral to the left, in the intermediate region between the posterior region and posterolateral region on the left, in the intermediate region between the posterior region and the posterolateral region on the right, anterolateral on the left, anterolateral on the right, in the intermediate region between the posterolateral region and the anterolateral region on the right, in the intermediate region between the posterolateral region and anterolateral region on the left, central. In case of petrification of the tumor more than 40 percent relative to its volume, palliative surgery is performed in the presence of one of the options for the location of the basilar artery relative to the tumor: posterior, posterolateral on the right, posterolateral on the left, in the intermediate region between the posterior region and the posterolateral region on the left, in intermediate region between the posterior region and the posterolateral region on the right, anterolateral on the left, anterolateral on the right, in the intermediate region between the posterolateral region and the anterolateral region on the right, in the intermediate region between the posterolateral region and the anterolateral region left, center.
EFFECT: method makes it possible to increase the radicality of removal of neoplasms of the skull base, spreading into the ventral parts of the posterior cranial fossa, and to reduce the incidence of severe complications.
