FIELD: obstetrics and fetal surgery.
SUBSTANCE: simulation of intrauterine embolization of Galen vein aneurysm (GVA) is carried out using microcoils. The right atrium of a fetal sheep is used as a model of Galen vein aneurysm. In this case, premedication is performed, in which, 20 minutes before the start of the operation, aminazine 2.5% - 2 ml, diphenhydramine 1% - 2 ml, and xylanite - 1.2 ml are administered intramuscularly at the rate of 0.25 per 10 kg of body weight. Next, a 2% lidocaine solution in a volume of 2 ml is injected into the sacral space on the right and left. Every 1.5 hours of the operation, a 2.5% solution of aminazine is administered in a volume of 2 ml and a 1% solution of diphenhydramine in a volume of 2 ml. Under ultrasound control, the left atrium is punctured into the fetus with an 18G needle. Through the lumen of an 18G needle, 2 or more detachable microcoils with a coil diameter of 10 mm and a length of 40 cm are placed in the form of a tangle into the cavity of the left atrium. At each stage, Doppler measurements of blood flow through the atrium and valve are performed.
EFFECT: method makes it possible to practice intrauterine surgical skills of partial occlusion of the GVA in order to reduce blood discharge and relieve cardiovascular failure using experimental animals and microcoils.
