FIELD: medicine; thoracic surgery.
SUBSTANCE: in the phase of the wound process of inflammation with exudation to the bottom of the wound over the entire area, extending within 5 mm, a paraffin dressing is placed on the bony edges of the wound. A perforated underwater catheter with 3 holes is installed under the paraffin dressing from the upper edge to the middle parts of the wound. The catheter is closed at the outer end. A foam sponge with a drainage catheter running through the center and having 6 holes is placed over the paraffin dressing. The top of the dressing is sealed with an impermeable film. Using a drainage catheter in the wound, a negative pressure of 90 to 120 mm Hg is created. Perfusion of the antiseptic is carried out daily with continued vacuum aspiration. A mixture of 3.0 ml of 3% hydrogen peroxide and 17.0 ml of 0.9% saline is used as an antiseptic. Using two or three 20.0 ml syringes, an antiseptic is injected into the underwater catheter. The vacuum dressing is changed in the operating room after 3 days and every 3 days thereafter. Perfusion therapy is completed when the wound enters the second phase of the wound process. Vacuum therapy is continued as an independent treatment.
EFFECT: method makes it possible to reduce the inflammation phase of the wound process and the overall time required to prepare the wound for reconstructive plastic surgery of the chest wall for sternomediastinitis.
