FIELD: medicine; cardiovascular surgery.
SUBSTANCE: invention can be used for stenting an isolated lesion at the mouth of a side branch.
Bifurcation stenosis 0-0-1 according to the Medina classification of one of the coronary arteries is detected. The diagnostic catheter is changed to a 6 Fr guide catheter. Next, the first coronary conductor is inserted into the distal part of the affected side branch, the second coronary conductor is inserted into the distal part of the main artery, from which the affected side branch arises. The stent is then inserted into the proximal segment of the affected side branch, and a balloon catheter of comparable diameter to the diameter of the main artery at the origin of the affected side branch is inserted into the main artery from which the affected side branch arises, so that the middle of the balloon catheter corresponds to the origin of the affected side branch. The stent is then positioned at the ostium of the affected side branch so as to completely cover the atherosclerotic plaque, and the stent is implanted at the ostium of the affected lateral branch. Next, the balloon catheter of the stent delivery system is pulled proximally by 5 mm, and balloon angioplasty is performed using the kissing technique of the main artery, from which the affected side branch arises, and the affected side branch in order to direct the prolapsed stent beams in the area of the carina bifurcation longitudinally in relation to the main artery.
EFFECT: method allows to perform effective stenting in isolated lesions of the mouth of the side branch without compromising the main artery.
