FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to endocrinology, and can be used to determine the probability of ACTH-ectopic syndrome (ACTH-ES) in patients with ACTH-dependent endogenous hyperadrenocorticism. Complex of diagnostically significant parameters is determined: ACTH blood level in the morning (pg/ml), blood potassium level (mmol/l), daily urine free cortisol level (nmol/day), level of cortisol in blood in the evening (nmol/l), level of cortisol in a sample of saliva collected in the evening (nmol/l), maximum diameter of a pituitary adenoma (mm). Set of obtained values is then processed using a classification model trained to determine the probability of the presence of ACTH-ES in the patient, expressed as a percentage. Gradient boosting model — Generalized Boosted Modeling, GBM, can be used as the classification model. Maximum diameter of a pituitary adenoma can be determined by means of MRI of the brain with intravenous contrast enhancement.
EFFECT: method enables determining the probability of ACTH-ES in patients with endogenous hyperadrenocorticism for the purpose of differential diagnosis of Itsenko-Cushing's disease and ACTH-ES by determining blood levels of: ACTH, potassium and cortisol levels: in blood, urine, saliva, and pituitary adenoma diameter, which leads to a decrease in the pharmacological load, consisting in the complete elimination of the need for the patient to use high doses of dexamethasone within the large dexamethasone test, as well as to complete elimination of invasiveness of differential diagnostic measures, to simplification of differential diagnosis of ACTH-dependent hyperadrenocorticism by reducing the need and reducing the number of patients referred for bilateral selective blood sampling from inferior petrosal sinuses.
