FIELD: medicine; cardiology; functional diagnostics; oncology.
SUBSTANCE: echocardiographic examination is performed using the speckle tracking technology, when using the ultrasonic sensor connected to the ultrasonic diagnostic system during the patient examination mark down on the area of apical impulse, obtaining and sequentially recording video loops of complete cycle of LV contraction in two-, three- and four-chamber apical positions under ECG control. Thereafter, the recorded video loops are analyzed in a semiautomatic mode, and the doctor marks the LV endocardium-blood boundary in the area of interest in accordance with the algorithm laid down in the ultrasound device. Ultrasound diagnostic system then analyzes the global longitudinal deformation of the LV and generates polar diagram of all 17 analyzed segments of LV myocardium in form of "bovine eye" with indication of deformation value in each segment. At that, cardiotoxicity is determined with reduction of LV deformation in all its basal segments along the circumference of the outer part of the polar diagram of GLS LV > −18 %, with compensatory hyperkinetic contractility of apical segments—inner part of GLS diagram LV < −18 %, as compared to values of global longitudinal deformation of LV in this patient, determined initially, before beginning of cardiotoxic chemo- or target therapy.
EFFECT: method increases accuracy and enables to detect cardiotoxicity at the stage of preclinical changes using a non-invasive speckle tracking technique, and to make timely decision on temporary suspension or change of chemotherapeutic treatment regimen, and also allows to expand possibility for differential diagnosis of reasons causing reduction of myocardial contractility.
