FIELD: medicine.

SUBSTANCE: invention relates to medicine, namely to neurosurgery and vertebrology, and can be used for sacroiliac joint denervation. Method involves skin puncture under X-ray navigation, followed by electrical stimulation to identify branches innervating the sacroiliac joint, and radiofrequency neuroablation. Skin is punctured below the lower edge of the sacroiliac joint for insertion of the electrode. Puncture is made with a flexible electrode, which is then advanced parallel to the sacroiliac joint at 3–5 mm medial to the joint space to the upper edge of the sacroiliac joint. Electro-stimulation to identify branches innervating the sacroiliac joint is followed by radiofrequency neuroablation. Thereafter, the flexible electrode is pulled down parallel to the sacroiliac joint at 3–5 mm medial to the joint space. That is followed by electrical stimulation to identify the branches innervating the sacroiliac joint and radiofrequency neuroablation. Cycle of said operations is repeated until the working part of the flexible electrode covers the area of the branches innervating the sacroiliac joint throughout the sacroiliac joint. After that, RFNA is carried out using a standard method at temperature of 80–90 °C with exposure for 90–180 sec.

EFFECT: method provides an increase in the destruction zone, possibility of destruction of branches L4, L5 of roots, reduced radiation load on medical personnel and patients, better tolerance of the procedure by the patient due to peculiarities of denervation.
