FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to dentistry, and is intended for use in simultaneous volume replacement of lost bone tissue and attached gums. Cone-beam computed tomography is carried out followed by computer image processing, where the bone resorption value in the area of teeth and implants is determined. Patient’s gingival phenotype and the depth of periodontal pockets are clinically determined for patient verification. Further, the patients undergo infiltration and conduction anaesthesia in the area of vertical bone defects. Split flap is formed, which is then connected to the laxative incisions. Entire thickness of the flap is detached apically from the vertical bone defect. After mobilization of the flap, granulation tissues and subgingival dental deposits are removed with curettes. EDTA solution is applied on the exposed root surface. A cortical bone plate is perforated with a spherical bur to form a blood clot; an osteoplastic xenogenic material is laid, the granules are covered with a resorbable membrane. Further, infiltration anaesthesia is performed in the area of connective tissue graft sampling from hard palate. In the donor area, haemostasis is performed and sutures are applied. Connective tissue graft is de-epithelized and fixed in the area of bone defects to the periosteum, partially overlapping the collagen membrane resorbed by Resopren 5/0 monofilament suture with U-shaped sutures, overlapping the bone granules. Mobilized split flap in the area of surgical intervention is placed in place, fixed with interrupted and blanket U-shaped sutures with a suture material.
EFFECT: method allows reducing injuries during surgical intervention and reducing postoperative complications.
