FIELD: medicine.
SUBSTANCE: invention relates to ophthalmology. First stage involves selective laser trabeculoplasty using a Q-switched, Nd: YAG laser at wave length 532 nm. Laser applicates with diameter of 400 mcm are applied on the pigmented trabecular membrane along a section of a circle corresponding to segment from 90° to 360° in amount of 50 to 100 depending on the length of the segment, pulse duration is 3 ns, laser exposure energy in range from 0.3 to 1.4 mJ. Then intraocular pressure (IOP) is monitored for next 6 months with interval of 1 month and with increase in IOP over 20 mm Hg, second stage of treatment is carried out, at which micropulse cyclophotocoagulation is performed with laser with wavelength of 810 nm, wherein the micropulse laser sensor is placed on the conjunctiva in the pars plana projection, perpendicular to the sclera, and without removing the sensor from the conjunctiva, laser irradiation is performed on areas of circle from 4 to 6 hours. Then successively on areas of circle from 6 to 8 o’clock, from 10 to 12 o’clock, from 12 o’clock to 2 o’clock, at the same time on area of 3 o’clock and 9 o’clock the laser is not exposed, at laser radiation power 1200–2000 mW, pulse duration 0.5 ms, period of 1.1 ms, duty ratio is 31.3 %, time of sensor passage along one area of circumference is 10 sec, at that in each of the specified areas the exposure is repeated 5 times.
EFFECT: method allows to achieve stable hypotensive effect with preservation of visual functions at glaucoma stage I-IIIB-C.
