FIELD: medicine, ophthalmology.
SUBSTANCE: transscleral cyclophotocoagulation is performed contact-transsclerally using a diode laser in micropulse mode with a wavelength of 810 nm and an exposure of 80 sec, along a circular arc 160 degrees long in the upper hemisphere of the eyeball, 4 mm from the limbus, with an energy of 1,200 mJ, with a duty cycle of 31.3%, pulse duration of 0.5 ms, and period of 1.1 ms. One hour before the micropulse transscleral cyclophotocoagulation, selective laser trabeculoplasty is performed with the following parameters: wavelength of 532 nm, spot diameter of 400 μm, exposure of 3 ns, energy of 0.7–2.0 mJ, while the laser beam is focused through the side mirrors of the Goldmann lens on the surface of the trabecula, at the level of the Schlemm's canal projection. Laser is exposed in 55–70 single appliques, 180 degrees long in the lower sector, at an equal distance from each other.
EFFECT: method makes it possible to preserve visual functions, activates uveoscleral and trabecular outflow tracts of intraocular fluid, achieves a hypotensive effect, which makes it possible to achieve a reduction in the hypotensive regimen, up to its cancellation.
