SUBSTANCE: invention refers to medicine, namely to oncology. Prior to chemoradiation therapy, high-resolution anorectal manometry is performed. Preoperative course of chemoradiation is performed, 8 weeks after its completion, high-resolution anorectal manometry is repeated, followed by a surgical intervention in the volume of a low anterior rectal resection with a preventive ileostomy exteriorization. When the level of maximum compression pressure is reduced by 30% of the initial value and more and increasing the volume of the first sensation by 60% or more according to high-resolution anorectal manometry data 8 weeks after completion of chemoradiation therapy ileostomy is closed for up to 2 months after a low anterior rectal resection with a preventive ileostomy.

EFFECT: method enables to reduce the incidence of manifested low anterior resection syndrome (LARS) in the patients by closing the ileostomy for up to 2 months after the low anterior resection with preventive ileostomy, which will improve the social adaptation of patients; provides fast recovery of anorectal function in postoperative period.
