FIELD: medicine.
SUBSTANCE: invention relates to medicine, namely to traumatology and orthopaedics, elbow surgery, and can be used for elbow arthroplasty in osteoarthrosis of I-II degree, chondromatosis, arthrofibrosis, heterotopic ossificates. Method of elbow arthroplasty in osteoarthrosis of I-II degree, chondromatosis, arthrofibrosis, heterotopic ossificates includes dorsal longitudinal approach, preparation of soft tissues, formation of a penetrating hole between a coronal fossa and an olecranon fossa, removal of bone elements, cicatricial elements of a capsule and ligaments through the penetrating hole. Soft tissues are prepared at the level of epicondyles of humerus along an elbow joint capsule to form lateral canals. Anterior portion of the elbow joint is approached from top to bottom, through a penetrating hole between a coronal fossa and an olecranon fossa and lateral canals formed at the level of epicondyles of the humerus, along the capsule of the elbow joint. Bone elements, cicatrices, ligaments are removed by flexion and extension of the elbow joint. Volume of the defect in the loaded part of the articular surface of the humerus is determined. Cartilage is sampled from the unloaded part of the articular surface in the area of the penetrating hole between the coronal fossa and the fossa of the olecranon of the humerus, forming an autograft, which is laid in the defect of the loaded part of the articular surface of the humerus, covering the entire surface of the defect, and impacted in the defect area. Control flexion and extension of the elbow joint are performed. Method of elbow joint arthroplasty in osteoarthrosis degree I-II, chondromatosis, arthrofibrosis, heterotopic ossificates is implemented on modern equipment, using modern technologies and materials.
EFFECT: advantages of the proposed method in comparison with existing ones consist in the fact that arthroscopic approaches of a large size; in addition to the penetrating hole between the coronal fossa and the fossa of the olecranon, lateral canals are formed, at the level of the epicondyles of the humerus, which makes it easy to get medical instruments, expands the range of actions and performs cleaning of large bone elements, cicatricial elements, capsule elements and ligaments from the penetrating hole and lateral canals, this leads to complete cleansing and a high-quality result; low risk of injury; range of motion in the elbow joint is restored by placing the autograft into the defect in the loaded part of the articular surface of the humerus.
