FIELD: medicine; cardiosurgery.
SUBSTANCE: puncture is performed by supplying and receiving instruments from the epicardial surface of the left ventricle wall within its apex. Feeding and receiving tools are introduced into the valve area located behind the prolapsing part of the flap. Artificial filament is fed by means of a feeding tool into the valve area located behind the prolapsing part of the flap. Free end of the artificial suture coming out of the feeding instrument is captured in the left atrium by means of a loop grasping of the receiving instrument. Said end of the artificial suture is brought out onto the epicardial surface of the left ventricle. Ends of artificial suture are fixed with tension and formation of new pair of valve chords, wherein after removal by means of receiving instrument of said end of artificial suture on epicardial surface of left ventricle end of the feeding instrument is displaced under the valve flap parallel to the flap edge at distance of 5–10 mm with repeated puncture of same flap 5–10 mm from the point of the first puncture. Further, the second end of the artificial thread is pushed through the feeding tool into the loop of the receiving tool, then the second end of the artificial suture is brought out onto the epicardial surface of the left ventricle. After the both ends of the artificial suture are brought out onto the epicardial surface of the left ventricle, they are passed through the sleeve, which is then inserted into the canal formed in the wall of the left ventricle after removing the receiving instrument. Thereafter, the ends of the artificial suture are passed through the support plate installed on the side of the epicardial surface of the left ventricle, and the ends of the artificial suture are tightened on the surface of the support plate to form a pair of valve chords retaining the prolapsed portion of the valve leaf. In a particular case, if it is necessary to create more than one pair of chords, all the ends of the filaments are first brought out onto the epicardial surface of the left ventricle, and then passed through the bushing.
EFFECT: method enables reducing injuries and increasing the strength of fixation of artificial filaments forming chords on the epicardial surface of the left ventricle.
