FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to oncology and dermatovenerology, and can be used for treating stage 0-IIc extremity skin melanoma. Skin is marked; an irregular ellipse is formed around the tumour; the marked flap is excised to an underlying muscular fascia along Langer's lines and along a lymphatic drainage pathway. In the perpendicular direction on both sides of the axis of the ellipse, there is an indent from the edge of the tumour according to the Breslow thickness: 0.5 cm – for melanoma in situ, 1.0 cm – for tumour thickness ≤ 2 mm, 2.0 cm – for tumour thickness > 2 mm. Marking and excision along Langer’s lines is performed in distal direction. Marking and excision along the lymph outflow direction is performed in proximal direction 2-2.5 times longer than in the distal direction, so that the tumour remains located on the distal one-third of the removed flap. Wound is closed with linearly interrupted or cosmetic sutures.

EFFECT: method enables increasing the survival rate of the patients by capturing transistor metastases, excising the affected area along Langer's lines and reducing the length of the operation.
