FIELD: medicine.
SUBSTANCE: invention relates to cardiology. Patients undergo verification of non-ischemic aetiology of cardiomyopathy, standard 12-lead electrocardiography, according to the results of which a notch or notch of the QRS complex is determined in the thoracic lead V5, reflecting an increase in the time of electrical activation of the walls of the left ventricle, as well as transthoracic echocardiography, according to the results of which, the ratio of the diameter of the sinus of Valsalva to the body surface area and the rate of active filling of the left ventricle are determined, reflecting aortic diameter at the level of Valsalva sinuses and left ventricular diastolic dysfunction, respectively. A probability of a positive response to cardiac resynchronization therapy is then calculated using a predictive model, created based on the formula of the logistic regression equation and the collected data. Formula is calculated using the ratio of sinus diameter to body surface area and the rate of active filling of the left ventricle, presence of non-ischemic aetiology of cardiomyopathy and notch or notch of QRS complex in thoracic lead V5. If the prognostic coefficient is more than 0.57, a positive response to cardiac resynchronization therapy is predicted in the patient.
EFFECT: method makes it possible to increase efficiency of cardiac resynchronization therapy in patients with chronic heart failure, and also allows to optimize selection of patients-candidates for implantation of cardiac resynchronizing device, which reduces number of irrationally implanted devices, thereby improving patient’s quality of life and optimizing economic costs.
