FIELD: medicine.

SUBSTANCE: invention refers to medicine, namely to ophthalmology. Before surgery, anaesthesia with Sevoflurane is introduced, then epiglottis air duct I-gel is installed, further anaesthesia is maintained at level of 1-1.2 MAC with control of the level of consciousness with BIS-index of 65-70, intraoperative administration of Paracetamol solution in dosage 15 mg/kg. Further, the surgical intervention is performed in 2 stages, in the first stage after corneal de-epithelisation, instillation of Dextralink solution is started. Then, a femtosecond laser interface is installed, an intrastromal annular tunnel is formed, which is filled with Dextralink solution using a cannula, wherein one or two segments with thickness of 250 or 300 mcm are implanted into the tunnel, respectively. Further, the supply of the inhalation anaesthetic is reduced to 0.5-0.8 MAC with constant monitoring of the central position of the eyeballs and monitoring the level of consciousness with BIS-index of 75-85 and subsequent gradual transition to spontaneous breathing, and stroma impregnation is continued for up to 30 minutes. At the second stage, the patient’s cornea is exposed to ultraviolet light for 10 minutes with power of 9 mW/cm2 with installation of Dextralink solution.

EFFECT: method enables reducing the time and number of surgical interventions, thereby reducing the number of intraoperative complications, stopping the progression of keratoconus, improving and fixing the refractive properties of the cornea, as well as improving the quality of life by rehabilitation of visual functions.
