FIELD: medicine.
SUBSTANCE: group of inventions relates to medicine, namely to traumatology and orthopaedics, and can be used for arthroscopic suturing of the knee joint meniscus. Surgical instrument is used for arthroscopic suturing of the meniscus of the knee joint, consisting of a handle with a cannula, from which a working part of the guide wire departs in the form of a cannulated needle with an inner diameter of 1 mm with backward bent 180° conically pointed end, handle is made of heat-resistant plastic, and working part is made of medical stainless steel. Instrument is inserted into a joint cavity through a standard anteromedial port, bending parallel to the tibial plateau, horizontally towards a posterior horn of the medial meniscus, and passing between the articular surfaces of the femoral condyle and tibia. Then the instrument is delivered into the posterior medial section above the posterior horn of the meniscus under the obligatory control of the arthroscope, after visualization of the instrument in the posterior medial section, it is turned by 90° in such a way that the tip can be stuck under the meniscus rupture from the side of the ramp throughout the thickness. Entire thickness of the meniscus ramp is punctured by traction from the joint under visual control. Through the reverse guide, the guide wire with the loop is delivered into the anterior medial portion of the joint; the free end of the guide wire with the loop is brought out from the joint cavity through the anteromedial port. Suture is threaded into the loop; the traction of the free end of the guidewire is used to pull the suture through the reverse guide. Under the control of the arthroscope, the guide is punctured from the meniscus ramp and removed from the joint, while the suture remains in the joint. Free ends of the suture are brought out through the anteromedial port; the knot is fixed by means of a nodal pusher, thus forming a blanket suture of the posterior meniscus horn. Joint is sanitized, wounds are sutured in layers, and an aseptic dressing is applied. Surgical instrument for arthroscopic suturing of a knee joint meniscus consists of a handle with a cannula, from which a working part of a guidewire departs in the form of a cannulated needle with an inner diameter of 1 mm with a bent in the opposite direction under 180° conically sharpened end, the handle is made of heat-resistant plastic, and the working part is made of medical stainless steel.
EFFECT: group of inventions allows minimizing the risk of neurovascular damage to the articular cartilage, and also provides matching the edges of the rupture for further healing with controlled tension, which enables to preserve meniscus mobility, start earlier rehabilitation, minimize suture cutting.
