SUBSTANCE: invention relates to maxillofacial surgery, oncology. Marking is applied along the edge of the hair growth zone of the temporal region, continuing through the apex of the tragus, fringing the earlobe, passed into the behind-the-ear region and turned on the neck in the region of the anterior edge of the sternocleidomastoid muscle. Skin is incised. It is mobilized 2 cm anteriorly from the anterior edge of the masseter. Infratemporal fossa fat is visualized. Superficial muscular-aponeurotic system of the face is dissected along an anterior edge of the masseter to a base of the lower jaw. Visualization and neurolysis of the buccal and zygomatic branches of the facial nerve are performed. Facial fascia and masticatory fascia are dissected. After traction of the masseter muscle, a front edge of a mandibular ramus is visualized. Masseter muscle fibers are subperiosteally separated to visualize the posterior edge of the ramus of the lower jaw. Coronoid process is visualized, and the coronal process is resected along an oblique line.

EFFECT: method enables to provide access to the infratemporal fossa without damaging the nerve fibers of the facial nerve, as well as preserving the functional state of the muscles of the neck and face, that in the remote postoperative period solves the problem of aesthetics and functional state of the maxillofacial region.
