SUBSTANCE: invention relates to maxillofacial, reconstructive, aesthetic surgery. After dissection of skin and subcutaneous fat, dissection of skin-fat flap is performed from upper edge of incision to lower edge of parotid salivary gland. A posterior edge of the sternocleidomastoid muscle is visualized with a greater auricular nerve, which is lateralized to an anterior edge of the sternocleidomastoid muscle. A superficial musculo-aponeurotic system and a subcutaneous muscle of the neck are plicated and fixed with interrupted sutures to a periosteum of a zygomatic arch. Excess superficial muscular-aponeurotic system and subcutaneous muscle of neck are dissected, and skin is excised. Before skin excision, skin is mobilized from an underlying superficial musculo-aponeurotic system and a subcutaneous muscle of the neck.

EFFECT: method allows to compensate for the defect in the parotid-masticatory region without damaging the nerve fibers of the greater auricular and facial nerves, as well as preserving the functional state of the neck muscles, that in the remote postoperative period solves the problem of aesthetics and functional state of soft tissues of the parotid-masticatory region.
