FIELD: medicine.
SUBSTANCE: invention refers to medicine, namely to surgery and neurosurgery, and can be used for percutaneous discectomy with the help of a guide probe in case of lumbar intervertebral disc herniation. X-ray controlled puncture is performed with lateral inclination of 50° to the frontal plane of the body and at distance of 10-12 cm from the line of spinous processes. Under X-ray control in frontal and lateral projections, the cannula is advanced into the intervertebral disc. After the fibrous ring is punctured, the guide stiletto is removed from the cannula, and a discography is performed. After controlling the position of the distal portion of the cannula, the sliding limiter of the cannula is brought to the puncture point of the patient’s skin, the guide stiletto is removed, and a screw stiletto of the discectomy instrument is inserted into the cannula. Nucleus pulp substance of the intervertebral disc is collected by moving the screw stylette with the cannula in an anteroposterior direction by 1 cm and turning it along the axis of the cannula. Then, without going beyond the fibrous ring, the direction of movement of the cannula is changed by puncture of the intervertebral disc at angles of 30° and 70° to the body surface in the frontal plane.
EFFECT: method provides reducing the number of injuries and labour intensity of the surgical intervention by using a single skin puncture allowing access to the desired intervertebral disc without excessive injuries of soft tissues.
