SUBSTANCE: invention relates to medicine, specifically to ophthalmology. What is involved is a combined laser treatment of primary open-angle glaucoma with the laser exposure covering a trabecula in two stages. First stage involves exposure of trabecula zone in upper, upper temporal and upper nasal parts of angle of anterior chamber, where the pigmentation of the structures is weaker, along a circular arc of 180 degrees, with laser radiation by means of a first YAG laser with wavelength of 1064 nm with single pulses in amount of 50-55 at an equal distance from each other, radiation energy 0.9-1.0 mJ, spot diameter 10 mcm, exposure 3 ns until microcavitation bubbles appear. Immediately after this, at the second stage, selective laser trabeculoplasty is performed in the inferior, inferior nasal and inferior temporal parts of the angle of the anterior chamber, where the pigmentation of the structures is more intense, along a circular arc of 180 degrees by means of a second YAG laser with wavelength of 532 nm by non-overlapping laser applicates in amount of 65-75, radiation energy 0.5-0.8 mJ, spot diameter 400 mcs, exposure 3 ns, in the threshold mode until microcavitation bubbles appear.

EFFECT: method provides higher clinical effectiveness in glaucoma of the eyes with uneven pigmentation of the structures of the anterior chamber angle.
