FIELD: medical science.

SUBSTANCE: invention refers to medicine, namely to surgery. After lobe mobilization with transection of superior and inferior thyroid vessels, recurrent and irrecurrent laryngeal nerves are searched by staged zonal dissection of paratracheal subcutaneous fat with using microsurgical instruments. Isosceles obtuse triangle is formed on the wound bottom in the paratracheal region on the side of the operation being performed. Vertex of the triangle lies in the laryngeal-tracheal angle formed by conditional lines drawn along a lower border of the larynx and along an outer contour of the trachea. Base of the triangle is an arbitrary line drawn through the points arranged on the sides of triangle at 2 cm from the laryngeal-tracheal angle. Then in the triangle the internal RLN-zone and the external IRLN-zone are selected. That is ensured by dividing a triangle base into three equal portions, and a line parallel to an external border of the larynx is drawn from a point found at the interface of the middle and lower thirds in accordance with Fig. 6. An area of Berry's ligament and a junction of recurrent and irrecurrent laryngeal nerves into the larynx are localised. Recurrent laryngeal nerve is found first in an internal RLN-zone starting from a base of the triangle to a Berry's ligament.

EFFECT: method makes it possible to facilitate the search and reliable identification of the recurrent and irrecurrent laryngeal nerves during surgical operations on the thyroid gland, to increase the safety of surgical intervention, and can also be used as a simple and visual aid for training surgeons to safe and reliable surgical tactics without injuring the recurrent and irrecurrent laryngeal nerves.
