FIELD: medicine, esophageal surgery. SUBSTANCE: one should carry out superior median laparotomy. Left hepatic lobe should be mobilized. Hernial gastric and esophageal parts are descended under diaphragm. Their posterior walls should be mobilized. Diaphragm is exfoliated from pericardium at 5.0 cm width to the front against it. One should suture anterior cardial wall under the edge of hernial sac and diaphragm to the front against esophageal opening for 4.0 cm. Cardia should be replaced into the niche developed. Cardia is fixed in the niche. Lateral gastric walls should be sutured with diaphragmatic pedicles. The method enables to develop antireflux valve. EFFECT: higher efficiency of operation. 6 dwg
