SUBSTANCE: it is necessary to evaluate the duration of the disease, previous therapy, the level of basic pressure in anal canal in projection of internal sphincter, average pressure in anal canal at volitional contraction and the presence of chronic hemorrhoid to calculate the point index. Moreover, the duration of the disease being up to 1.5 mo - 1 point, 1.5-3 mo - 2 points, 3 mo-1 yr - 3 points, above 1 yr - 4 points. While evaluating the previous therapy, 1 point corresponds to no therapy at all, 2 points - at temporary, instable effect after conservative therapy, 3 points - at no effect after conservative therapy, 4 points - at the relapse after surgical treatment. While evaluating the level of basic pressure in anal canal in projection of internal sphincter 1 point corresponds to the pressure ranged within the norm, that is 49-67 mm mercury column; 2 points - at the increase of pressure by 15-30% against the norm; 3 points - at the increase by 31-50% against the norm; 4 points - at the increase being above 50%. While evaluating the average pressure in anal canal at volitional contraction 1 point corresponds to the pressure within the norm, that is 58.8-94.4 mm mercury column; 2 points - at the increase of pressure by 15-30% against the norm; 3 points - at the increase by 31-50% against the norm; 4 points - at the increase being above 50%. While evaluating the presence of chronic hemorrhoid 1 point corresponds to no hemorrhoid or chronic hemorrhoid stage 1; 2 points - at chronic hemorrhoid stage 2; 3 points - at chronic hemorrhoid stage 3; 4 points - at chronic hemorrhoid stage 4. Then points should be summarized. At point index ranged 8-11 points it should be prescribed the dissection of fissure without sphincterotomy. At point index being above 11 points - the dissection of fissure in combination with the dosed sphincterotomy. The innovation enables to standardize the indications for matching the tactics of treating anal fissure, improve the results and decrease the number of complications and relapses of the disease in question.
