SUBSTANCE: before operation one should detect the type of patient's constitution and basal pressure in anal canal in projection of internal sphincter, Anal fissure should be dissected. At basal pressure in anal canal in projection of internal sphincter ranged 60-120 cm water column one should dissect the sphincters longitudinally up to the upper edge of anorectal ring along inter-sphincter groove in patients of dolichomorphous type of constitution at the part of 3-5 o'clock of conditional clock-face, in patients of meso- and brachymorphous type of constitution - at the part of 4-6 o'clock At basal pressure in anal canal in projection of internal sphincter being above 120 cm water column in patients of all the types of constitution one should additionally dissect the sphincters longitudinally up to the upper edge of anorectal ring along inter-sphincter groove from contralateral side at the part of 9-7.5 o'clock of conditional clock-face. The innovation enables to achieve adequate denervation of internal anal sphincter that removes hypertonicity of internal anal sphincter, prevents the spasm as a response to defecation, favors the healing of post-surgical wounds, decreases the intensity of post-surgical pain syndrome and early rehabilitation in patients, excludes the lesion of the branches of superior rectal arteries, doesn't deal with transverse dissection of the fibers of anal sphincters that prevents their failure.
