SUBSTANCE: invention concerns medicine, surgery, gastroenterology and can be used for tactics choice of diaphragmocruroplasty at surgical treatment of hernias of esophageal aperture of diaphragm. At carrying out of intraoperative tool inspection of the patient the distance size d (cm) between diaphragm legs in a standard place for back crurography is defined, indicator F (kPa) of tension force of diaphragm legs and value of factor of a strain (K) is calculated under the formula: K=F/d. At value K>0.7, back crurography is carried out. At value 0.3≤K≤0.7, back crurography with diaphragmocruroplasty a mesh explant of the triangular form is carried out. At value K<0.3, diaphragmocruroplasty using a mesh explant of the rectangular form without back crurography is carried out.
EFFECT: method provides reduction of traumatism operative measures, frequency of postoperative complications and disease relapses.
