METHOD OF CHOOSING LAPAROSCOPIC CLOSURE TECHNIQUE IN PERFORATED GASTRIC AND DUODENAL ULCER Russian patent published in 2012 - IPC A61B17/00

Abstract RU 2451491 C1

FIELD: medicine.

SUBSTANCE: invention refers to surgery and is applicable for choosing an laparoscopic closure technique in perforated gastric and duodenal ulcer. Perforated ulcer is classified, namely: the perforation diameter ≤2 mm makes stating class I, >2 mm and ≤5 mm - class II, >5 mm and ≤10 mm - class III, >10 mm - class IV; the measured extent of inflammatory infiltrate ≤5 mm makes stating class A, >5 mm and ≤10 mm - class B, >10 mm - class C. The stated classes are used to choose laparoscopic closure technique in perforated ulcers. Classes IA, IB or IIA requires the opening to be closed with one z-shaped absorbable suture; classes IC or IIB involves the opening to be closed by separate single-layer absorbable suture, while classes IIC, IIIA or IIIB enables the opening closed by separate double-layer suture: a first layer is absorbable, while a second one is non-absorbable; class IIIC, IVA, IVB or IVC perforated gastric ulcer has the opening closed by separate double-layer suture: a first layer is absorbable, while a second one is non-absorbable; if observing class IIIC, IVA, IVB, IVC duodenal ulcer, laparoscopic closure of the perforation is not considered to be indicated.

EFFECT: method enables reducing a risk of complications related to an inadequate choice of a surgical technique.
