FIELD: medicine.
SUBSTANCE: method involves Seldinger's puncture, plan heart radiography, coronary angiography, X-ray controlled application of diagnostic and ablation electrodes. Endocardial electrophysiological examination, RF-exposure, follow-up coronary angiography and test endocardial electrophysiological examination are also performed. Right and left cubital veins are punctured to approach an endocardial cavity. 5Fr Introducer is inserted into the right vein, whereas 7Fr introducer is inserted into the left vein; thereafter, a diagnostic electrode at an electrode distance of 2-5-2 or 2-2-2 mm is brought through the right introducer into the right ventricular cavity; a 20-pole diagnostic electrode is inserted through the left introducer into a right ventricular outflow tract. A left radial artery is punctured according to Seldinger's technique; 6Fr introducer 23 cm long is inserted into the left radial artery, and a solution of myotropic antispasmodic agents is introduced. That is followed by arteriography of the radial artery; 6Fr introducer 23 cm long on a conductor is replaced by 7Fr introducer 45 cm long. An ablation electrode is then brought into the left ventricular cavity.
EFFECT: method enables providing early patient's activation, preventing complications associated with transfemoral arterial and venous approaches, such as retroperitoneal hematoma, false aneurism, arteriovenous fistula, as well as thrombophlebitis and main vein thrombosis associated with using introducers of greater diameter.
