SUBSTANCE: invention refers to medicine. A surgical instrument comprises a pivotal system with a rotary drive element, which interacts with a pivotal transmission. Rotating the drive element in the first rotation direction about a driving axle makes a surgical end effector rotate pivotally in the first pivotal rotation direction under the action of at least one pivotal element. Rotating the drive element in the second rotation direction makes the surgical end effector rotate pivotally in the second pivotal rotation direction.
EFFECT: surgical instrument provides a better access to tissues to be incised and sutured in a tight space.
