FIELD: medicine.

SUBSTANCE: prior to the surgery, the patient is subject to computer tomography. Computed Tomography Data are converted to STL format. A bone implant is simulated with individual parameters in the volume of the alveolar bay and bone window of the maxillary sinus anterolateral wall with subsequent milling of the previously lyophilized bone biomaterial. The mucous membrane is incised; a mucoperiosteal flap is dissected and elevated under local anesthesia. A bone window is created using burs, drills, taps, the maxillary sinus mucosa alveolar bay bottom is peeled. The milled implant from pre-lyophilized bone biomaterial with dental implant bone guides in the volume of alveolar bay and bone window of the maxillary sinus anterolateral wall are placed in the surgery area under the mucosa. The individually milled bone implant is fixed due to anatomical retention to the bay alveolar and the bone window and of the maxillary sinus anterolateral wall, as well as dental implants mounted on the milled bone guides considering the individual anatomical and topographic features of the patient. The wound is closed.

EFFECT: method allows to reduce the error in bone-plastic material positioning in the maxillary sinus bottom area, to optimize the step of material introduction into the maxillary sinus, to shorten the period of implanted material integration, to reduce the risk of material dislocation and the risk of maxillary sinus mucosa perforation.
